Informed Consent

 

I understand that Reiki is a complementary healing practice and is not a substitute for medical treatment. I consent to receive this Reiki treatment and acknowledge that the practitioner will use hands placed on or near my body during the session (depending on my comfort level) as well as tools such as but not limited to: sound healing, essential oils and sprays, crystal healing, and possible smoke clearing, based on intuitive guidance and individual preferences. I understand that I have the right to speak up if something is making me uncomfortable and also have the right to change my mind. I understand that the results of Reiki sessions may vary and that the practitioner cannot guarantee specific outcomes. 

 

I understand that Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that Reiki can compliment any medical or psychological care I may be receiving but is not a replacement for medical or psychological care. 

 

Confidentiality Agreement

 

I understand that all information shared during the Reiki session and on the intake form will be kept confidential. If needed, the practitioner will make referrals to other suitable providers when appropriate, necessary, and possible. 

 

Release of Liability

I release the Reiki practitioner from any liability for any adverse reactions or outcomes that may result from the Reiki session. The energy channeled during Reiki is spiritually intuitive; thus, the practitioner is not liable for any emotional, mental, physical or spiritual discomfort experienced by recipients during or after a Reiki healing session.
